This looks like this was supposed to be closed vault but for some reason its habitants were roaming free. There was hellephant which felt quite early for Vaults:1. I think it was shapeshifter larping water nymph, that broke vault monsters free.
after you finish the game please post the morgue here so we can check if there was a digging enemy or the vault is messed up!
