I am interpreting this as you wish to know wish to know what happened but don’t wish to hear advice.
You got petrified by a basilisk. The petrification takes a precious few turns to take effect but once it does you’re essentially paralyzed but with a little extra AC. It’s usually not a death sentence but you have very low AC, your main defense (shields) was disabled, and you were out in the open and got surrounded by 8 enemies so they could all hit you. (actually 9 enemies, snapping turtle with reaching attacks)

I’ve taken a look at the ttyrec. I’m not sure I would have moved far away from the stairs to melee with a horde of yaks in view and more monsters being alerted constantly by the noise, especially once a yak turned up behind me.
Once that had happened, though, I’m afraid not one but two basilisks appearing (along with globs of other monsters) and only MR+ was a crisis which wasn’t recognised.
- It’s safer to do Lair:5 after clearing Orc
- Heavenly Storm is designed for situations like this yet you haven’t used it; you also had tremorstones
- Never stay in the open with an enemy in sight who can petrify/paralyze you - scroll of fear could help you reposition
