Mutagenic Glow is contamination, right? Two hits of Irradiate (from Radroach) can give you severe contamination. Or some of that contamination could have been self-inflicted.

I wondered about the radroach, but the description for Irradiate doesn’t say anything about mutations. Aside from drinking a mut pot, how would one self-mutate?
contamination can cause mutation though.
If you cast high fail-rate spells repeatedly, it can happen. Used to be a common worry for transmuters and invisibility specialists. (Because invisibility would also gradually contaminate you.)
You can still force it with high fail-rate spells, with radroaches, and with mutagenic fog.
If you want mutations you can also follow Xom.

Unequipping something with ^Contam is also enough to give you severe contamination.
If you’re seeking mutations, contamination is probably not a good choice, since it’s skewed towards giving you bad mutations.
